Wound treatment from start to end

We know very well from all our experience that every wound is different and they require skilled intervention to be treated. We therefore offer the best wound care at home, suitable to your personal needs and situation. After you have been registered with us, either directly by yourself or by your practitioner, we will contact you within 24 hours to schedule a home visit. We believe it is important that we during the first home visit to be able to speak to your family care giver, so that we can get to know each other better and also hear their story. We see the client and / or the / family caregivers as a partner in de care teams.

After the intake interview, the wound specialist will work with you to determine the wound treatment plan or a wound care plan based on the advice of your main practitioner. With us, wound care starts right from the first visit.

The wound care plan

Our wound care plan contains several elements, from the description of the wound to the wound care materials, a step-by-step wound care plan and the frequency of wound care. Our wound assessments don’t only look at the wound itself, we take other aspects into account, such as your diet and degree of mobilization. In order to properly describe your general health situation, we use NANDA (North American Nursing Diagnosis Association) classification and associated nursing diagnoses.

Using the NANDA diagnoses, ARA Wond & Zorg Wound Specialist determines interventions and expected outcomes targeting your health status and wound healing process. If you need other (temporary) care or nursing in addition to wound care, we can of course also help you with this.

Digital file

Your data and information about your healthcare need are securely stored in our digital client files. In this digital file, the wound status, wound characteristics, progress of wound healing and the tasks and responsibilities of the wound care providers involved are accurately recorded. Our wound specialists carefully describe all steps of wound care in our wound registration form so that the wound is cared for according to the fixed agreements.

During the treatment we frequently take pictures of your wound, in order to monitor the healing process accurately. The photos are stored in your personal digital file. Once a week we evaluate the condition of your wound together with you and / or your caregiver to monitor the healing process and adjust the treatment plan if necessary.

We regularly send an overview of your healing process to your primary practitioner so that your general practitioner or specialist in the hospital has a complete and up-to-date insight into the progress of the healing process.

For your own use, a 'folder' will be left at your home so that you or your caregiver will always be informed about your wound healing process.
